Some poorly children can leave hospital early, and get cared for at home.
Special nurses from the PATCH team can visit your home and care for you outside of hospital.
Sometimes, you can even avoid going to hospital completely! The idea is to make sure you only go to hospital if you have to.
PATCH nurses can come to you home, and do tests to make sure you’re getting better. They can also give you the right medicine, and they are always in touch with doctors about the best care for you.
